
As your Case Manager RN, I will serve as the central point of coordination and support for you and your family throughout your healthcare journey. I will assess your medical, emotional, and social needs, help you to understand your condition and carefully create a care plan, collaborate with your physicians, therapists, and other multidisciplinary providers to ensure safe, effective treatment. I advocate for your goals and preferences, assist with navigating community resources, and help prevent gaps in care by organizing services such as home health, equipment, follow-up appointments, and education. My role strengthens communication, reduces stress for you and your family, and promotes better outcomes by ensuring that your care is personalized and aligns with your specific needs. It is my hope that we form a long-lasting relationship, where we both teach and learn from each other. It is my privilege to be of service to you.

BIOGRAPHY
I am a graduate of Colorado Christian University with a Bachelor of Science in Nursing. I have over 15 years of experience in nursing, in almost every department imagineable! The past 10 years have been primarily focused on Geriatrics and Case Management, which allows me the precious gift of getting to know you, instead of the constraints of being tied to a typical "10 minute visit" in the clinic or hospital. My motto: "We are all in this together, so we need to take care of each other and take the time to truly listen."
In my personal life, I have also been a touring singer/songwriter, and still enjoy creating beautiful & powerful musical compositions. I live in the Texas hill country with my husband, cat, and dog.
